Signs You Might Need a Root Canal

The phrase "root canal" makes a lot of people immediately tense up. But the reputation is largely undeserved β€” root canals done with modern techniques are no worse than getting a filling. The problem is that the signs of needing one are easy to dismiss, and waiting makes everything worse.

What a root canal actually is

When the soft tissue inside a tooth (the pulp) becomes infected or inflamed β€” usually from deep decay, a crack, or repeated dental work on the same tooth β€” it needs to be removed. A root canal cleans out the infected tissue, disinfects the inside of the tooth, and seals it so the infection can't return. Then the tooth is capped with a crown. The tooth stays in your mouth, functions normally, and you don't feel it anymore.

The procedure itself is done under local anesthesia. Most patients are surprised by how much less uncomfortable it is than they expected.

Warning signs to take seriously

  • Persistent, severe toothache β€” especially pain that throbs or wakes you up at night. Normal tooth sensitivity after temperature doesn't qualify. We're talking about pain that doesn't go away.
  • Sensitivity to heat or cold that lingers β€” brief sensitivity is normal. If hot or cold food triggers pain that lasts more than 30 seconds after you stop eating, the pulp may be affected.
  • Swelling or tenderness in the gums β€” especially a raised, pimple-like bump near the affected tooth. That's often an abscess β€” the infection trying to drain.
  • Darkening of the tooth β€” a tooth that's become noticeably darker than its neighbors may have dead or dying pulp inside.
  • Pain when chewing or pressing on the tooth β€” especially if biting down sends a sharp jolt through the tooth.

What if there's no pain?

This is the tricky part. In some cases, a tooth that needs a root canal stops hurting β€” not because it's better, but because the nerve has died. The infection can still be spreading. This is why dental X-rays matter even when nothing hurts.

What happens if you wait

Infected pulp doesn't heal on its own. Left untreated, a dental infection can spread to the jaw, neck, or even beyond. You go from needing one procedure to potentially needing extraction plus a bridge or implant β€” a much bigger, more expensive outcome. The tooth can often be saved with a root canal done in time. Once the infection spreads far enough, saving it becomes much harder.
